Student Physicals
Student physicals for work, camp or sports can be obtained through the student's primary care physician or the Springfield Schools Health Center, normally by appointment only, please call (541) 682-3550.
Sports Physicals
All students participating in district-sponsored sports teams (middle and high schools) must have a sports physical on file in the school office.
Sports physicals are required every two years with a yearly update signed by the student's parent or guardian.
The Oregon School Athletic Association (OSAA) requires that sports physicals are only accepted from a licensed physician, licensed naturopathic physician, licensed physician assistant, certified nurse practitioner or a licensed chiropractic physician who has clinical training and experience in detecting cardiopulmonary disease and defect.
The physical MUST be completed before the student begins practice.
Students aged 15 and up are not required to have a parent present at the appointment but students must bring the appropriate form that indicates parental consent.
The Springfield Schools Health Center may be able to bill OHP Insurance; however, the initial examination fee for private insurance is $50.00. Contact your school athletic director for information on financial assistance that is available to help cover this cost.
