Dental Screenings
State law now requires a child who is 7 years of age or younger to have a dental screening before entering school for the first time. HB 2972 (2015). Springfield Public Schools partners each year with the Community Health Centers of Lane County to perform a quick, visual dental screening for all students in the district in kindergarten, first and second grades to help comply with this state law.
If you wish to opt your student out of this visual dental screening, please download, print and completely fill out the form below. Once completed, please return to the health aide at your student's school.

Dental Hygiene Services
Twice during the school year, the Community Health Centers of Lane County offer dental hygiene exams, teeth cleaning and fluoride varnish treatments, sealants and temporary sealants to all elementary and middle school students.
Authorization forms are sent home at the beginning of the school year and collected at the end of September.
These forms can be turned in at any time and the student will be included in the next round of services.
Contact your school's health aide if you need an authorization form.
They do attempt to get reimbursement from your insurance carrier whenever possible.
